Social Workers

VCH Social Workers provide clinical, consultative and professional health care services to people of all ages across the life span. Social Workers work from a client-centered, strengths-based and anti-oppressive perspective to partner with clients to enhance their social and psychological functioning and outcomes. Social Workers work within the framework of the social determinants of health and strive to address the physical, social, emotional, economic and systemic barriers that impact health and wellbeing.

Social Workers work within a variety of health-care areas including large urban settings and rural centres, such as:

  • Home and community care
  • Hospital-based medical, surgical and mental health units
  • Emergency
  • Long-term care
  • Primary Care Network and Urgent Primary Care Centres
  • Outreach on community-based mental health and substance use teams

Interventions you may be involved with as a Social Worker at VCH

  • Conducting psychosocial assessments and developing clinical care plans
  • Providing psychoeducation and interventions
  • Providing supportive counselling or psychotherapy
  • Coordinating individualized goal-oriented care
  • Providing resources and education
  • Helping individuals and families navigate service delivery systems

Social Worker - New grad program

VCH is pleased to offer new graduate social work support and learning opportunities to help you enhance your confidence and competence as you transition to independent practice. As a new graduate, you will have access to on-call support with clinical skills and decision-making, new grad education sessions and supernumerary buddy shifts to support your learning needs.

Our support team for new graduates includes access to a dedicated new grad SW educator in addition to the entire practice team of managers, practice leaders, site educators, and, of course, your co-workers.  

New Grad SW Educator can provide:

  • On-call support with clinical skills and decision-making, resource finding, and emotional well-being
  • Hands-on assistance with direct patient care
  • Education on social work foundational skills
  • Booked time for clinical support, mentorship, and career planning.

VCH’s New Grad Program includes:

  • Up to 140 supernumerary hours to supplement classroom learning and enable consolidation of knowledge.
  • Access to regional social work new grad educator to support onboarding, orientation, and continued education.
  • New Grad Community of Practice
