Harm reduction: Toward the Heart 

This service provides a support role for adults aged 25 or older with a primary diagnosis or concern is severe substance dependency. Clients may also have concurrent anxiety, depression, psychosis, fetal alcohol spectrum, developmental or mood disorders.

About qathet ICMT

The qathet ICMT team includes nursing support, occupational therapy and outreach workers.

 

 

Urgent Homelessness Response Team (UHRT)

This UHRT team is connected to the ICMT and works with clients with mental health and substance use concerns who find themselves homeless or precariously housed. This service is not available to clients who are not already followed by ICMT. 

This team includes a housing care coordinator, SWI and an Indigenous Homelessness Outreach Worker.  Hours are Monday to Friday, 8 a.m. to 5 p.m.

How to access

  • Check eligibility

    • Must be 25 years of age or older.
    • Serves community members whose primary diagnosis or concern is severe substance dependency.
    • Clients may also have concurrent anxiety, depression, psychosis, fetal alcohol spectrum, developmental or mood disorders.
    • Client needs exceed those that can be met by standard case management. Have little access to existing service/resources due to barriers (transportation, housing, income, mental/physical health).

Intensive Case Management Teams (known as ICM or ICMT) provide a support role for people who experience severe substance use challenges, chronic unmet medical needs, and multiple barriers to accessing services.
