Adolescent and Young Adult (AYA) Outpatient Program

The Adolescent and Young Adult Program provides youth ages 12 and older with acquired or congenital disabilities living in British Columbia and the Yukon an opportunity to address their rehabilitation needs and assistance with the transition to adult services.

How to access

  • Check eligibility

    • Adolescents 12 years old and older
    • Services are available to individuals living in every B.C. health authority, including Vancouver Coastal Health Authority, Fraser Health Authority, Island Health Authority, Interior Health Authority, and Northern Health Authority.
    • This program provides interdisciplinary assessment and treatment for persons with complex neuro-muscular-skeletal conditions.
    • Examples of conditions we see:
      • Moderate to severe brain injury
      • Spinal cord injury and spina bifida
      • Cerebral palsy
      • Neuro-muscular diseases such as Duchenne’s Muscular Dystrophy

  • Get a referral

    A referral from a doctor or nurse practitioner is required.

  • Information and referral to community resources
  • Education for clients, families, care providers and school

Team members mainly work Tuesday and Thursday but clerical and school are open Monday to Friday.
