Personalized Support & Stabilization Team Plus (PSS+) provides additional support in your home if your activity levels or independence have changed after returning home from the hospital.

The PSS+ team will work with you to develop a plan of care specific to you and your situation to help you return home confidently, regain your independence and function, and remain medically stable.

The program is short-term, and you and your clinician determine the length after an assessment in your home.

What to expect

  1. You and your hospital team identify if PSS+ might be a good fit to support your care needs.
  2. You will discuss with your hospital team what is important to you and develop a plan for going home.
  3. The PSS+ team will support your discharge home and coordinate with your hospital team to ensure a smooth transition home.
  4. Once you are home, your PSS+ team will work with you to develop a care plan to support you in resuming your daily activities. This will look different for each person and include:
    • Supporting you in your home
    • Setting specific goals with you and ways to achieve them
    • Ongoing assessment and care
  5. Once you have completed the program, your PSS+ team will, if required, help connect you to other community supports as needed.
